A school-based physical therapist is needed to provide specialized telehealth services for a 9th-grade student currently placed in a facility in Salt Lake City, UT. This remote role requires one hour of PT services per week, focusing on supporting the students physical therapy goals through virtual sessions. Service delivery via telehealth allows the therapist to practice remotely while holding a valid Utah PT license.

Key Responsibilities:

• Conduct weekly one-hour telehealth physical therapy sessions tailored to the individual needs of a high school student.
• Collaborate with educational and medical professionals to align therapy goals and ensure continuity of care.
• Maintain accurate documentation and progress notes in compliance with professional and regulatory standards.
